Why you should read this consent form

Jewish House (ABN 19 350 397 588) (we, our or us) collect and use your personal (health) information (such as your name and address and details about any relevant medical conditions) to better enable you to participate in our programs of support at the One&All creative arts hub (One&All Hub). This is personal information, so it is important that you understand what we do with it.

Why do we collect and use your personal (health) information?

In a nutshell, we collect and share the information to enable you to participate in programs of support at the One&All Hub.

We may share the information with a support worker (if you ask us to provide one for you), our staff or volunteers working at the One&All Hub, or a health care service provider in the event of an emergency.

 How is the privacy of your personal (health) information protected?

We keep all your personal (health) information secure, whether it is held in hard copy or electronically. Our computers are username and password protected and only authorised staff members have access to personal information. Information that is held in hard copy is held in a lockable cabinet.

 Can you withdraw your consent?

Yes, at any time by notifying us in writing at privacy@oneandallhub.org.

We and members of our team (staff, support workers and volunteers) at the One&All Hub are seeking your consent to collection, handling and limited disclosure of personal (health) information. Handling of personal (health) information about you by us is regulated by Australian Federal, State and Territory privacy and health data privacy laws.

This consent relates to personal (health) information about you, being:

  • the person providing this consent, or

  • the person on whose behalf this consent is being provided, if this consent is provided by a legal personal representative for someone else.

By acknowledging your acceptance of the terms of this consent form, you consent to collection, handling and limited disclosure of personal (health) information as explained below.

  • assessing your eligibility for our program of support,

  • providing a safe and accessible environment at the One&All Hub, and

  • enabling you to participate in programs of support at the One&All Hub.

For this purpose we may exchange personal (health) information about you with a support worker (if you ask us to provide one for you), our staff or volunteers working at the One&All Hub, or a health care service provider in the event of an emergency.

We collect your personal (health) information in a number of ways, including through intake forms on our website at www.oneandallhub.org, face to face intake assessments, and when you phone or email us for information.
